Payment & Insurance Options
Patients with Insurance
Our office gladly accepts and works with most dental insurance plans. Some companies we work with are Aetna, Ameritas, Assurant, Blue Cross Blue Shield, Cigna, Delta Dental, Guardian, Humana, Lincoln Financial, Metlife, Paragon, Principal Life, United Concordia and United Healthcare. We are in-network with Delta Dental but may be out-of-network with other providers, please call if you have any questions.
Patients without Insurance
We accept Mastercard, Visa, American Express, and Discover credit cards as well as cash, personal checks, and Care Credit. Please contact us if you have any questions or concerns.
Meet Our Team
Dr. Janette Dunivan
Dr. Dunivan graduated from Creighton University School of Dentistry and went on to complete an Advanced Education in General Dentistry residency, where she further developed her skills and experience in comprehensive patient care. After working in private practice, she made the decision to serve at a tribal dental clinic where she became the dental director, overseeing the operations of the dental department while continuing to see patients full time. This experience strengthened her commitment to providing compassionate, high-quality dental care while also teaching her the importance of building lasting relationships with patients and the community.
Her goal is to create a dental home where patients feel welcomed, respected, and cared for. She looks forward to providing you and your family with personalized, comprehensive dental care for many years to come.

What to Expect
Whether your first appointment is for a dental checkup, a consultation, second opinion, or emergency care, we do all we can to make your visit a welcoming, informative, and productive experience.
Once we gather the relevant background information, examine your smile and obtain any diagnostic images, we’ll discuss if treatment is required and explain your best options in care.
Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll discuss your dental concerns, collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.
New Patient Forms
You can save time and expedite your first visit by downloading and completing the forms linked below at home. Simply hand the forms to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.
